Social Rx is fully interoperable with SystmOne, so that SystmOne users can easily refer patients into Social Rx via a click-through URL link in the clinical system. Data recorded in Social Rx is also filed back into SystmOne.

Setting Up Interoperability 


Step 1: In SystmOne, go into the Setup menu item and then click Users & Policy and then URL & Program Maintenance


 

Step 2: Click on New URL which will pop up a window to create the URL details



 

Step 3: Enter the following details to create the URL:

 

Name:                Social Rx Referral

Description:       Link to the Social Rx Referral form

Launch in:          Leave it set to default browser

 

Encryption parameters:

IV:                       c4277aebd3201a816bf04d73e71d3c1d

 

Add fields step by step so that they contain the following:

Step 4: Test the URL by pressing the Test button.  This will pop up a populated referral form (if you were in a patient when you were adding the URL). If the form pops up and has some populated data, everything is working as expected. Click ok to complete the URL setup. Depending on the setup at your practice, the 'Test URL' button may not work. If it doesn't work at this stage, please try to load the referral form when you are in the case of a real patient. 


Note: If you have completed the steps above and would like to try with a test patient, please note that it may not work if your test patient does not have an NHS number.











 

Add Referral Form Link Button for a User



Step 1: Right click beside toolbar and select Configure Toolbar


 

Step 2: If you want to add the button to an existing set of buttons, select 'Amend Button Set'. If you would like a new row of buttons, select 'New Button Set'.


 

Step 3:Search for URL in the available button search field on the pop up. This will return a list of available URLs.  Double click on Social RX Referral.

 

 

Step 4: Choose the button style that you would like: Small; Large (with text); or Large (without text).


Step 5: Click ok and the button will appear in your toolbar.

 

 


 Please ensure that the URL button is added to the toolbars of all staff who may send referrals to social prescribing.







How to Refer a Client into Social Rx


Step 1: Search for and select a client in SystmOne.

 

Step 2: Click the Social RX Referral button to launch a pre-populated referral form.

 

Step 3: Fill in the form with the required details for the client. Please note, not all of the fields are pre populated and will require some extra mandatory information.

 

Step 4: Once you are happy with the details, submit the form to pass the referral into Social Rx Connect.

 

 

How data flows back into SystmOne


Data flows back to SystmOne after a number of actions on Social RX Connect.  Each of the options below can be configured to be on or off:

  1. If a client rejects and is discharged from Social prescribing
    1. An email is sent into the GP Practice mailbox stating the client has declined with the decline SNOMED code (871711000000103).  This message is picked up by the daily process and pushed into SystmOne.
  2. If the referral is accepted by the social prescribing team and client
    1.  The SNOMED code for social prescribing acceptance will be added (871731000000106). 
  3. If you complete an assessment for a client.
    1. An email is sent into the GP Practice mailbox stating assessment type and overall score.  This message is picked up by the daily process and pushed into SystmOne.
  4. If you complete an action plan for a client.
    1. An email is sent into the GP Practice mailbox stating an action plan has been completed.  This message is picked up by the daily process and pushed into SystmOne.
  5. If you refer a client onto a service provider.
    1. An email is sent into the GP Practice mailbox service selected and the booking date. This message is picked up by the daily process and pushed into SystmOne.
  6. If you discharge a client from Social Rx.
    1. An email is sent into the GP Practice mailbox stating that the patient has been discharged from Social Prescribing and it also contains the discharge reason.
