Our reports contain tabular data which give you the ability to analyse and drill down to individual records. 

They can be found via the side navigation bar under 'SRx Reports'.


As a link worker, you will only be able to view data for the patients that you are the assigned to. A link worker manager, however, will be able to see the data of all patients within the system. 


 

Types of Reports


We currently have the following reports within Social Rx: 


1. Outcomes Reports

The outcomes report delivers the outcome ratings that were entered in the Outcome Review for each patient. The report table contains all the patients who were referred into the Social Rx platform between the start and end date search parameters. You can change the dates and click 'search' to refresh. You can also filter each of the individual columns and group by a column by dragging a column header above the table. 



2. Assessments Reports 

This reports collates all the data from the assessments that are carried out with patients. You can select which type of assessment (ONS4. SWEMWBS, etc.) you want to view data for, and see individual patient scores and total scores. The last tab, titled 'SWEMWBS Assessment Overview' (yellow circle labelled '1'), shows you the percentage of patients whose assessment score improved, the percentage whose score declined, and the overall increase or decrease in wellbeing. This report is a great way of assessing how well your social prescribing programme is doing. 




3. Service Booking Reports

The service bookings reports displays all cases along with where each patient was referred. The second tab includes reason for referral in the report, too. The report table contains all the patients who were referred into the Social Rx platform between the start and end date search parameters. You can change the dates and click 'search' to refresh. You can also filter each of the individual columns and group by a column by dragging a column header above the table. 



4. Cases Reports

In this report you can filter cases by communication type, time spent, onward referrals, provider, referral status, NHS number or by SLAs.  You know also change the range of dates for which the data will be shown. 



5. Demographics Reports

By selecting different tabs (e.g. 'Age' on screenshot), this reports allows you to view demographic data including age, gender, employment status, ethnicity and more. 



When you hover over the tab you have selected, you will see two symbols appear. The first allows you to export the report to PDF, image or Excel. The second symbol allows you to maximise a table for that specific type of demographic data.


You can filter the data by date using the 'Period' section to the left. By clicking on the grey arrows, you can filter by month as well as year, as seen below in the screenshot. 


6. Referrals Reports

Here you can filter referrals by reason, source, GP surgery, month and demographics. You can also filter by date by using the drop-down calendar that appears when you click on the date. Once you've selected the start and end dates, select 'Search'.







Downloading Reports


Reports can be exported simply by clicking the blue button with the word 'PDF', 'Word' or Excel', depending on the format in which you would like the report to be downloaded.
