How to Refer a Client into Social Rx
Step 1: Search for and select a client in SystmOne.
Step 2: Click the Social RX Referral button to launch a referral form.
Step 3: Fill in the form with the required details for the client. Please note, not all of the fields are pre-populated and will require some extra mandatory information. We are currently working with the NHS Personal Demographic Service to populate this form with more details.
Step 4: Once you are happy with the details, submit the form to pass the referral into Social Rx Connect. There are two options for coding referrals: either the SNOMED code for social prescribing acceptance (871731000000106) will be added into the patient record when the referral is submitted, or, when the referral is accepted in Social Rx by a link worker. If you are unsure which setup you have, or to change your setup, please contact your Account Manager.
How data flows back into SystmOne
Data flows back to SystmOne after a number of actions on Social RX Connect. Each of the options below can be configured to be on or off:
1. If a client rejects or the referral is declined
a. A note goes into the client's SystmOne journal stating that the referral has been declined, and the decline SNOMED code (871711000000103) is added to the patient record.
2. If an assessment is completed for a client
a. A note goes into the client's SystmOne journal stating the assessment type and score.
3. If an action plan is completed for a client
a. A note goes into the client's SystmOne journal stating that an action plan has been completed.
4. If a client is referred onto a service provider
a. A note goes into the client's SystmOne journal stating the service selected and the booking date.
5. If a client is discharged from Social Rx
a. A note goes into the client's SystmOne journal stating that they have been discharged.
In addition to sending information directly into the patient record on SystmOne, Social Rx also populates a summary page which can be found by clicking the same Social Rx Referral button. This page gives a concise history of the client's social prescribing journey, including the referral date, social prescriber name, case notes, assessment scores, action plans and service booking information.
